October Vehicle Registration Renewals Alert: If your vehicle registration is up for Oct renewal, the postcard will say proof of insurance required. For 99% of residents, it is not. We have it in our records. Try to renew online or other method first as you should be fine if nothing has changed with your car insurance. See the Colorado Dept of Motor Vehicle statement for more information.

Disability Parking

Apply for a New Disability Placard or License Plate

1. Complete the application form (form DR 2219).

  • Requires signature of a licensed physician.
  • Anyone other than the applicant may witness the applicant’s ID information on the form.

2. Return the form

  • Option 1: Return the form online.
    • To complete the transaction online, you will need:
      • Secure and Verifiable ID (out-of-state driver’s licenses or IDs only accepted for short term placards)
      • For license plates: Payment for license fees and ownership tax
  • Option 2: Return the form by mail

  • Option 3: Return the form in-person to any county Motor Vehicle office in Colorado (does not have to be a Boulder County office)
    • The person applying must come in (or a designated Power of Attorney) in order to receive the placard on the spot. Otherwise, the placard will be mailed to the address shown on the application.
    • Appointments are not required for disability placards. All three Boulder County Motor Vehicle offices will prioritize people with disability placard applications, even as a walk-in.

Placards are free of charge. Short term placards are issued for 90 days.

Placard Renewal Process

All placard renewals can be completed online.

Temporary Placards cannot be renewed. You will need to apply for a new placard.

Renewing Permanent Placards or Plates:

  1. A notice will be mailed to the address on file when the placard/plate needs to be renewed. Complete the self-certification at the bottom of the renewal. (If this is your third renewal – year 9 – you will need to complete a new application).
  2. Return the notice online.You may also mail it, fax it, or come in.
