Long-Term Services and Supports help you with daily living activities like bathing, dressing, eating, housekeeping, meal prep, and medication management. Services are available at home, in assisted living, nursing homes, hospitals, care facilities, or through PACE, a comprehensive program for people 55 and older.

You Need Help With Daily Activities
A&I Avenues will perform a Functional Assessment — an assessment that determines what type of care you need. This shows whether Long-Term Services and Supports is right for you.

You Are 65 or Older, or You’ve Been Determined Disabled
You are 65 or older, or
You are under 65 and have been determined disabled by Social Security or our state contractor. If neither applies, you can submit a Medicaid Disability Application in English (Solicitud de discapacidad de Medicaid en español) for our state contractor to review. This review may take up to 90 days.
You Meet the Financial Limits
Income:
Your current monthly income is less than $2,982 a month. If your income is higher, additional steps may help you qualify.
Resources (savings and things you own):
- If you live alone: Your resources can’t exceed $2,000.
- If you’re married and your spouse lives at home: Your combined resources must be less than $164,660.
- If you and your spouse both need Long-Term Support Services:
- Living in separate nursing facility rooms: Your resources can’t be more than $3,000.
- Living in the same nursing facility room: Your resources can’t be more than $4,000.
Home and Community-Based Services
- Such as home safety, personal care, transportation, caregiver support, and meal preparation
PACE (Program of All-Inclusive Care for the Elderly)
- Helps older adults who need extra support stay safe and healthy
Facility-Based Care
- Assisted living
- Nursing homes
- Hospitalization
- Alternative care facilities

The income limit is $2,982 a month.
- For those who are over the income limit, an income trust may be established.
The individual resource limit is $2,000.
- If both members of a married couple are applying for long-term care benefits, their resource limit is $4,000.
- If both members are applying for long-term care benefits but living in separate rooms in a nursing facility, their resource limit is $3,000.
- The resource limit for the spouse of someone in a care facility (Community Spouse) is $162,660 (as of 2026).

A Functional Assessment is required for Long Term Care.
If you are currently on any Medicaid program, you can call A&I Avenues at 303-439-7011 to schedule an assessment for Long Term Care.
If you do not have Medicaid yet, please complete an application for Medical Assistance. Boulder County Human Services will then send a referral to A&I Avenues.
