There are many options available for submitting an application. You can apply online at Colorado PEAK . You can also contact Boulder County Housing and Human Services at 303-441-1000 or the Healthy Kids Initiative at 303-441-1589. Or, if you would like to come into the office, we have two locations. Do I need to change providers if I switched from Medicaid to CHP+?
Possibly, some providers accept CHP+ and Medicaid but others will only accept one or the other.Preparing for an Anthrax Emergency
