Choose ONE of the options below for submitting your Award Claim Form and IRS form W-9
(Option 1) EMAIL OR UPLOAD SCANNED FORMS ELECTRONICALLY
- Email the completed/scanned Award Claim Form and W-9 as attachments to grossreservoir@bouldercounty.gov
- Note: We only need the first page of the completed W-9, not the following pages of instructions
- Or, upload them through Boulder County’s forms upload application.
(Option 2) FILL OUT AND SUBMIT THE FORMS ELECTRONICALLY
- Fill out the W-9 electronically (save on your e-device and upload just the first page with your responses to the e-Award form)
- Upload the W-9 (first page only) and complete the Award Claim Form electronically using Boulder County’s e-Award form application.
(Option 3) MAIL THE COMPLETED (PAPER/PRINTED) FORMS TO BOULDER COUNTY:
