Use the following instructions when filling out the Absentee Affidavit – Application for Marriage License form. Additionally, please note that the absent party must fill this out completely in blue or black ink, have the affidavit notarized, and the other party must bring the affidavit and be present in order to obtain the license. A copy of the ID presented to the notary (proof of age and/or photo ID) of the absent party must also be presented with the affidavit. - “STATE OF COLORADO County of_____”
Enter the Colorado County in which the form will be notarized. If notarized in a different state, change “State of Colorado” to the correct state and provide the county where the signature will be notarized. - Legal Name
Enter applicant’s first, middle and last name. The name must be written exactly how it appears on the copy of the ID that is being provided to our office. - Previous Married Name
If applicant has been previously married enter married last name only.
