Nurse Care Coordination
The Children with Special Needs Program provides nurse care coordination. We work with families who live in Boulder County and have a child with a special health care need between the ages of birth and 21 years old.
Our program provides support, information, and connections to services.
Nurse Support Includes
- Monitoring development
- Helping you understand special health care needs and treatment choices
- Helping you to support your child within the health care system
- Guiding optimal growth and development
Our nurses are trained to be up to date on infant and child health care needs and resources.
Get Answers to Questions
- Accessing routine and specialty care
- Times of change and transition, such as Neonatal Intensive Care Unit (NICU) to home, to preschool, to kindergarten
- How your baby communicates with you
- Feeding and nutrition
- Infant and childcare
