Re-certification Packet

If you want to continue receiving benefits, you will need to recertify.

To recertify, you will need to:

  • Answer the questions in the re-certification packet that was mailed to you
  • Sign the packet
  • Date the packet with the date you completed it
  • Return it to us by due date

We will contact you if an interview is required. If you miss the appointment, you will need to reschedule and complete the interview or your benefits will stop.

You may need to give us proof of any changes in your home, money, bills you pay and things you own. To help us make a quicker decision, please send in proof with your re-certification packet. If anything is missing, we will tell you what we need and the date that it is needed by. If we don’t get the proof by the due date, your benefits may be late or may stop.

The information we are asking for may be used to approve your benefits for more than one program. In order to do this, it is important that you answer all the questions or mark that you do not have any changes. You also will need to sign and date the packet.
