Colorado PEAK is the place to apply for and manage your medical, food, cash, or other public benefits online.

Use this form if: You prefer a paper application and are applying for benefits for the first time (or adding a new program), including:

  • Food Assistance (SNAP, or the Supplemental Nutrition Assistance Program)
  • Medicaid (Health First Colorado)
  • Cash Assistance
    • Colorado Works/TANF (Temporary Assistance for Needy Families)
    • Adult Financial

Change Report Form

Use this form if: You need to report changes (for example: income, address, household members, or expenses) for public benefit programs, including:

  • Food Assistance (SNAP, or the Supplemental Nutrition Assistance Program)

  • Cash Assistance
    • Colorado Works/TANF (Temporary Assistance for Needy Families)
    • Adult Financial

Report required changes within the time limits for your program.

Child Care Assistance

Boulder County’s Child Care Assistance Program (CCAP) helps families with low incomes pay for child care for children from birth to age 12. If you qualify, CCAP will pay part of your childcare costs. Participants pay the rest of the costs based on their family size and income.

At this time, Boulder County CCAP is on an enrollment freeze. That means we are only accepting applications to add families to our waitlist. Current CCAP families can stay in the program if they still qualify and recertify every year.

To apply for the enrollment freeze, you can fill out the Prescreening Questionnaire to see if you might qualify. If you do, we’ll add your name to a waitlist.

If your family is already enrolled in Boulder County CCAP, you can stay in the program if you still qualify and fill out a Redetermination Form each year:
