Military Record Request

The Recording Division physical office is closed to the public. Online/phone services are available Monday-Thursday from 7:30 a.m.–5 p.m. Limited in-person services are available by appointment.

Military Record Request

A copy of a military record may be requested by completing a Request for Military Record–DD 214 Form . The form must be notarized.

Military records will only be provided to the following:

  • Veteran listed
  • Next-of-kin of a deceased veteran

Requests may be made by mail or email.
