The Long-Term Care Medicaid program covers various services, including care at home or in assisted living, the Program for All-Inclusive Care for the Elderly (PACE), and stays in a nursing home or hospital for more than 30 days.
Long-Term Care Medicaid
The income limit is $2,742 a month.
- For those who are over the income limit, an income trust may be established.
The individual resource limit is $2,000.
- If both members of a married couple are applying for long-term care benefits, their resource limit is $4,000.
- If both members are applying for long-term care benefits but living in separate rooms in a nursing facility, their resource limit is $3,000.
- The resource limit for the spouse of someone in a care facility (Community Spouse) is $148,620 (as of 2023).

Completing a Functional Assessment.
If you are currently on any Medicaid program, you can call Adult Care Management Inc (ACMI) at 303-439-7011 to schedule an assessment for Long Term Care.
If you do not have Medicaid yet, please complete an application for Medical Assistance. Boulder County Human Services will then send a referral to ACMI.
