Required Information for Awarded Vendors
You are not required to provide this information with a bid or quote; however, if you are awarded an opportunity, you must provide this information before performing work and in order to receive payment.
- Awarded vendors must provide the following information:
- Legal Name and Business Address
- Contact name and email for person who will sign contract
- Invoicing email and invoicing mailing address
- Contact name and email for project manager for the awarded opportunity
- Awarded vendors must provide the following documents, even if they have provided them to the County previously:
- Completed IRS Form W-9 signed and dated within the past 12 months
- State of Colorado Certificate of Good Standing for the Vendor Legal Name specified in item 1, and dated within the past 12 months. Refer to the Colorado Secretary of State’s FAQs on the Certificate of Good Standing.
- If you are an individual and have provided a social security number on your W-9, you do not need to provide a Certificate of Good Standing.
- If you are a business incorporated in a state other than Colorado, provide a statement of foreign entity. Refer to the Colorado Secretary of State’s Foreign Business FAQs.
- Vendor Sample Invoice
- Certificate(s) of Insurance that meets or exceeds the requirements in Exhibit A provided with the solicitation for the opportunity
