St. Vrain & Left Hand Water Conservancy District

Please complete the form below if:
- You are an owner of the property listed in the letter you received (or spouse or civil union partner)
- AND
- You are registered to vote at another address in Colorado that is outside the St. Vrain & Left Hand Water Conservancy District
You do not need to complete this form if:
- You are registered to vote at the property listed in the letter or at another address within the boundaries of the St. Vrain & Left Hand Water Conservancy District
- OR
- You are not registered to vote in the State of Colorado
To be sent a property owner ballot by mail, this form must be received by the Boulder County Elections Division by Monday, October 28. After October 28, you must come in-person to the Boulder County Elections Division or a Boulder County Vote Center to receive a property owner ballot.
Once eligibility is confirmed by the Boulder County Elections Division, a property owner ballot will be sent to you, which is separate from your general election ballot.
